(“AdaptHealth”) is a publicly traded medical equipment company. It recently experienced a data security incident, potentially exposing the sensitive information of an undisclosed number of AdaptHealth customers.

According to a Form-8K AdaptHealth filed with the Securities and Exchange Commission on July 2, 2026, a threat actor contacted the company on June 15, 2026. It claimed to have obtained data from AdaptHealth’s systems. AdaptHealth has since confirmed that some data—including a stored password file associated with insurance billing—was exfiltrated from its systems. The company also disclosed that the threat actor accessed certain external electronic health record system portals. Personally Identifiable Information and Personal Health Information may have been compromised in this data breach.

(“GDI”) is a provider of government services specializing in the vehicle emissions inspection and Fish and Game license sales industries. Texas Parks and Wildlife Department (“TPWD”) lists GDI as its current license sales contractor. TPWD recently disclosed that its licensing vendor experienced a data security incident, potentially exposing the sensitive data belonging to over 3 million Texas hunting and fishing license customers.

On an undisclosed date, Texas Cyber Command discovered a cybersecurity incident involving the TPWD license system vendor handling the sale of hunting and fishing licenses. An investigation revealed that an unauthorized actor may have obtained certain Texas hunting and fishing license customers’ Personally Identifiable Information (“PII”). The compromised PII includes, but is not limited to, driver license information, passport numbers (if provided), email addresses, phone numbers, and residential addresses.

On March 17, 2026, Assurance became aware of malicious activity by an unauthorized third-party on their internal servers that had taken place the day before, on March 16, 2026. After investigation into the attack, Assurance found that personally identifiable information (‘PII’) of hundreds of thousands of policyholders was potentially exposed. PII includes, but is not limited to, names, contact information, insurance account information, automobile insurance policy information, vehicle and driver information, claims-related information, driver’s license numbers, taxpayer identification information and Social Security numbers. Given the sensitivity of the information involved and the number of Assurance policyholders affected, with 611,046 South Carolinians affected alone, the impact of this breach is concerning. It recently experienced a data security incident, potentially impacting some DentaQuest clients’ sensitive data.

In May 2026, hacking/extortion group ShinyHunters listed DentaQuest on its Dark Web data leak site. According to an article by technology news outlet Bleeping Computer, ShinyHunters claims to have stolen more than 234GB of data from DentaQuest. ShinyHunters later published the stolen data, the article says. On June 2, 2026, DentaQuest confirmed a breach occurred. Data breach monitoring site HaveIBeenPwned reviewed the leaked data and reports that 2.6 million unique email addresses were compromised in addition to names, genders, dates of birth, health insurance information, government-issued IDs, phone numbers, and physical addresses.

According to Station Casinos’s listing on the portal, the breach occurred and was discovered on March 5, 2026. Station Casinos began sending consumer notification letters on May 21, 2026. Technology news outlet CyberNews reports a Station Casinos spokesperson confirming “that in March 2026, Station Casinos experienced a cybersecurity incident in which an ‘unauthorized third party’ accessed the systems. According to the company, attackers accessed a single employee’s account and associated files.” The Office of the Vermont Attorney General’s data breach portal describes the Station Casinos breach impacting Security Numbers.

An investigation revealed that—between December 10, 2025 and January 27, 2026—EFHC’s network was subject to unauthorized access. Data impacted by the breach includes some patients’ Protected Health Information (“PHI”) and Personally Identifiable Information (“PII”). It has over 185 locations throughout the U.S. A third-party vendor providing Lumexa with administrative support recently experienced a data breach, potentially exposing the sensitive information of some Lumexa patients.

On April 9, 2026, the vendor informed Lumexa that it was investigating suspicious activity in a section of its network containing Lumexa patient data. On April 15, 2026, Lumexa learned that an unauthorized actor may have viewed or obtained copies of documents with Lumexa patient information from the vendor’s network. Documents were taken between March 31, 2026, and April 9, 2026. Data impacted by the breach includes some patients’ Protected Health Information (“PHI”) and Personally Identifiable Information (“PII”). The affected PHI and PII includes, but is not limited to, names, identifiers such as dates of birth, addresses, phone numbers, Social Security numbers, and/or patient account numbers; insurance information; and clinical information including visit dates, diagnoses, or other health information related to radiology services.
