Baptist Medical Center, San Antonio, Texas, and Resolute Health Hospital, New Braunfels, Texas, announced a data breach affecting more than 1.2 million Texans alone. Individuals affected by the breach received services at one of their Texas locations.
According to the notification letter sent to affected persons, an unauthorized character accessed personal identifiable information (“PII”) and protected health information (“PHI”) on their network between March 31, 2022, and April 24, 2022; the access was discovered on April 20, 2022. Letters notifying affected parties of the breach were sent on or about June 17, 2022. The PII and PHI referenced in the notice includes full names, dates of birth, address, Social Security numbers, medical information (e.g., medical record number, dates of service, provider and facility names, chief complaint or reason for visit, and other visit, procedure and diagnosis information), billing and claims information (e.g., account and/or claim status, billing and diagnostic codes and payor information).
