The personally identifiable information (PII) and protected health information (PHI) of certain current employees and listed dependents/beneficiaries as well as individuals who sought treatment or services from Southwest Health Center was compromised due to a data breach. The breach was discovered on January 11, 2022 but was not reported to the U.S. Department of Health and Human Services (U.S. Dept. of H&HS) until July 5, 2022….the same date letters were sent to affected individuals and a post was created on the Center’s website. The compromised PII and PHI includes dates of birth, Social Security numbers, driver’s license or state identification card numbers, financial account numbers, medical information, and/or health insurance information. According to a notification submitted to the U.S. Department of Health and Human Services, 42,142 individuals were affected.
