Avamere Health Services Data Breach Investigation

Nearly 381,000 employees of entities utilizing Avamere Health Services’ information technology services had their personally identifiable information (“PII”) and protected health information (“PHI”) compromised during a data breach. It appears the affected individuals include former Avamere employees though this information is unconfirmed. Of the nearly 381,000 affected individuals, 183,000 are employees of Premere Rehab, LLC. Premere Rehab operates under the name “Infinity Rehab.” The exposed PII and PHI includes names, addresses, SSNs, DOBs, driver’s license numbers, claims Info, financial account numbers, medications info., lab results, and medical diagnosis/conditions info. The exposure of the PII and PHI occurred intermittently from January 19-March 17, 2022, and the breach was discovered on May 18, 2022. Avamere began notifying affected individuals by a letter dated June 17,2022 and reported the breach to the U.S. Dept. of Health and Human Services on July 13, 2022.
