First Choice Community Healthcare (FCCH) provides primary medical care, dental care, WIC services and integrated behavioral health care in and around Albuquerque, New Mexico. The health organization experienced a data breach that exposed names, Social Security Numbers, dates of birth, First Choice patient ID number, diagnosis and clinical treatment information, medications, dates of service, health insurance, info, medical record number, patient account number. Though the breach was discovered on March 27, 2022, impacted patients were not notified until on or about August 1, 2022.
