Fast Track Urgent Care Center is an associate of Synergetic Healthcare Solutions. In February 2022, Fast Track Urgent Care Center learned from its vendor, PracticeMax, Inc., that an unauthorized party gained access to its network in May 2021. The breach affected 258,411 individuals. The compromised information included the following types of personally identifiable information and protected health information: name, address, Social Security number, date of birth, treatment and/or diagnosis information, health insurance information and, in some cases, financial information. On June 10, 2022 PracticeMax sent notification letters to individuals. Synergetic notified the U.S. Department of Health and Human Services of the breach on July 12, 2022.
