Paylogix, LLC (“Paylogix”) is a Westbury, New Jersey-based company specializing in software solutions for insurance providers. It experienced a data security incident, potentially compromising the sensitive information belonging to an undisclosed number of individuals.
After experiencing a network disruption, Paylogix initiated an investigation of its systems. The investigation revealed that an unauthorized actor accessed and copied certain files from the Paylogix network between November 13, 2025 and November 18, 2025. Consequently, some individuals’ Personally Identifiable Information (“PII”) and Personal Health Information (“PHI”) were impacted. The compromised PII and PHI includes—but is not limited to— dates of birth, Social Security numbers, voluntary benefit information, access credentials, driver’s license numbers, electronic signatures, financial account information, financial account numbers, health insurance information, IRS PIN numbers, medical information, passport numbers, state identification numbers, taxpayer identification numbers, and U.S. alien identification numbers.
