Michigan Avenue Immediate Care (MAIC) is an immediate, urgent care center located in downtown Chicago, IL. MAIC experienced a data breach in which 144,104 patients’ personally identifiable information (“PII”) and protected health information (“PHI”) was compromised. The “PII” and “PHI” included names, addresses, Social Security numbers, dates of birth, health insurance information and medical history. The breach involved an unauthorized party accessing MAIC’s files between November 11, 2021, and May 4, 2022, with the access being detected on May 1, 2022. MAIC began sending letters to affected individuals on or about June 30, 2022. At the same time, a notification of the breach was submitted to the U.S. Department of Health and Human Services.
