Harris Center for Mental Health and IDD (Intellectual and Developmental Disability) Data Breach Investigation

Between November 6, 2023, and November 7, 2023, an unauthorized character gained access to the network of Harris Center for Mental Health and ID. The breach resulted in the compromise of personally identifiable information (“PII”)and protected health information (“PHI”) belonging to 238,463 individuals. The compromised information included the individuals’ name, address, phone number, Social Security Number, date of birth, email address, U.S. Alien Registration Number, driver’s license/state ID number, financial account information, treatment/diagnosis information, prescription information, provider name, medical record/case number, Medicare/Medicaid ID number, health insurance information, and treatment cost. The breach was discovered on November 7, 2023, and notification letters were sent to affected individuals on or about April 15, 2024.
