Episource, LLC Data Breach Investigation

Episource, LLC provides medical coding and adjustment services to health organizations including doctors, health plans and health companies. Between January 27, 2025 and February 6, 2025, Episource’s network was breached by an unauthorized hacker, compromising the personally identifiable information (“PII”) and protected health information (“PHI”) it received from its client health organizations. One known health organization is Sharp Community Medical Group, the largest physician network in San Diego. Episource discovered the breach on February 6, 2025, and determined that the following PII was compromised: patients’ and insureds’ names, addresses, Social Security numbers, dates of birth, phone numbers and email addresses, member/group ID numbers, Medicaid-Medicare-government payor ID numbers, medical record numbers, doctors, diagnoses, medicines, test results, images, care, and treatments. On June 6, 2025, Episource began sending notification letters to the individuals whose PII was compromised during the breach.
