Swift Institute (“Swift”) is a multi- specialty medical group comprised of spine, neurosurgery, orthopedic, vein and neurology specialists and has several locations in Reno, Sparks, Carson City and Elko, Nevada. On or about January 30, 2025, a breach of personally identifiable information (“PII”) and protectedl health information (“PHI”) occurred when an unauthorized actor gained access to Swift’s network. The breach compromised the PII and PHI of an undisclosed number of patients. According to a notice posted on Swift’s website, the PII and PHI included names, Social Security numbers, driver’s license or state identification card numbers, dates of birth, financial account information, credit or debit card information, medical information, health insurance information, and digital signatures. Though the breach was discovered on January 30, 2025, Swift did not begin mailing notifications to affected individuals until on or about December 10, 2025.
