Located in Birmingham, Alabama with offices in surrounding areas, Cardiovascular Associates experienced a data breach that affected 442,000 patients. The information that was compromised included names, addresses, Social Security Numbers, dates of birth, passport and driver’s license numbers, health insurance information, medical record numbers, dates of service, provider and facility names, other visit procedure and diagnosis information, credit and debit card information, financial account information, billing and claims information, billing and diagnostic codes, payor information, and possibly assessments, tests and imaging. The breach occurred between November 28, 2022, and the date the breach was discovered: December 5, 2022. Notification of the event was sent to patients on February 3, 2023, and posted on Cardiovascular Associates’ website the same day.
