Community First Medical Center (“CFMC”)Data Breach Investigation

On or about July 12, 2023, an unauthorized party accessed the computer systems of Community First Medical Center (“CFMC”), a Chicago-based hospital. The access was discovered on July 28, 2023, and “CFMC” began notifying the 216,047 individuals affected by the breach on September 26, 2023. As a result of the breach, individuals’ personal information was compromised including Social Security numbers.
