Cherry Street Services (dba Cherry Health) Data Breach Investigation

Cherry Health is a health center with operations across Michigan. Based in Grand Rapids, the health center is comprised of more than 800 health care professionals at 20 locations and provides primary care, dental, vision, behavioral health, and pharmacy services. On or about December 21, 2023, Cherry Health experienced a network breach that was discovered on December 24, 2023. Due to the breach, individuals’ personally identifiable information (“PII”) and protected health information (“PHI”) was breached including first and last name, addresses, Social Security numbers, dates of birth, phone numbers, health insurance information, health insurance ID number, patient ID number, provider name, service date, diagnosis/treatment information, prescription information, and financial account information. Notification of the breach was sent to affected individuals on or about April 16, 2024.
