Pacific Guardian Life Insurance (“PGL”) Data Breach Investigation

Unusual activity in the email environment of Pacific Guardian Life was detected on September 5, 2023. The detection led to the discovery of an August 25, 2023, data breach that compromised 167,103 individuals’ personally identifiable information (“PII”) and protected health information (“PHI”). The “PII” and “PHI” included individuals’ names, Social Security numbers, financial account information, payment card information, medical information, health insurance information, date of birth, driver’s licenses or state ID, US. Alien identification number, taxpayer ID, copy of marriage certificate, mother’s maiden name, and electronic signature. “PGL” began issuing notice letters to affected individuals on April 4, 2024.
