Superior Air-Ground Ambulance Service, Inc. (“Superior”) – the largest privately owned ambulance company in the Midwest – announced a breach affecting 858,238 individuals. Though the breach occurred between May 15 and May 23, 2023, the company did not issue its notification until nearly a year later, May 10, 2024. The notification states the breached files included personally identifiable information (“PII”)and protected health information (“PHI”) such as name, Social Security numbers, date of birth, address, driver’s license or state
identification number, financial account information, payment card information, patient record information, medical diagnosis or condition information, medical treatment information, and health insurance information. “Superior” services five states including Illinois, Indiana, Michigan, Ohio, and Wisconsin.
