Panorama Eye Care, LLD Data Breach Investigation

Nearly one year after the breach of its internal network, Panorama Eye Care began notifying individuals about a breach of their internal network. According to a June 5, 2024 notification letter, the breach occurred between May 22, 2023 and June 4, 2023 when an unauthorized actor gained accessed to its network. Personally identifiable information (“PII”) and protected health information (“PHI”) of 377,911 individuals was compromised because of the breach. The compromised “PII” and “PHI” included address, telephone number, date of birth, Social Security number, driver’s license number, driver’s license state, military identification number, passport number, Alien Registration Number, bank account number, routing number, credit and/or debit card number, expiration date, security code/PIN number, medical history, lab results, retina scans, prescription information, treating/referring physician name, patient number, medical treatment information, medical diagnosis information, medical record number (MRN), health insurance information, Medicare or Medicaid number, username and password, email address/username and password, security question and answer.
