One Point Patient Care is a hospice pharmacy benefit manager (“PBM”) with nearly 100 locations throughout the United States. “OPPC” experienced a data breach between August 6 and August 8, 2024. The breach was detected on August 8, 2024; however, the company did not announce the breach on its website until October 14, 2024. Personally identifiable information (“PII”) and protected health information (“PHI”) were exposed during the breach, and the “PII” and “PHI” included customer names, Social Security numbers, residence information, medical record number, diagnosis, and prescription information. It is assumed affected individuals will receive a notification in the mail.
