Set Forth, Inc. and Centrex Software, Inc. Data Breach Investigation

According to reports filed with several state attorney generals’ offices, 1.5 million individuals had their personally identifiable information (“PII”) compromised because of a data breach involving Set Forth, Inc. and Centrex Software, Inc. The date of the breach was not disclosed in the reports; however, May 21, 2024 is the date the breach is said to have been discovered. It was not until on or about November 8, 2024 that Set Forth and Centrex Software notified the impacted individuals. The individuals were mailed letters stating their “PII” and possibly the “PII” of their spouse, co-applicant, and dependent was compromised; and, the “PII” included names, Social Security numbers, dates of birth, and addresses.
