Hillcrest Convalescent Center, Inc. (“Hillcrest”) Data Breach Investigation

Hillcrest Convalescent Center, Inc. (“Hillcrest”) offers post-hospital rehabilitation, 24/7 skilled care, assisted living, home health and outpatient therapy in Raleigh and Durham, NC. On an undisclosed date, an unauthorized character gained access to Hillcrest’s network, and the access was discovered on June 27, 2024. Beginning on or about March 3, 2025, Hillcrest began notifying approximately 106,194 individuals that their personally identifiable information (“PII”) and protected health information (“PHI”) had been compromised. The PII and PHI included name, date of birth, Social Security number, patient data, medical information, treatment information, health insurance information, and health care provider information.
