Alternate Solutions Health Network (“ASHN”) partners with healthcare providers by providing home health programs and hospice care to patients. A list of some of the 44 hospitals and home health programs partnering with ASHN is provided below. On or about May 30, 2024, an unauthorized character gained access to an ASHN email account. The breach resulted in the acquisition of 93,000 patients personally identifiable information (“PII”) and protected health information (“PHI”). The PII and PHI included patients’ names, Social Security numbers, dates of birth, addresses, driver’s license numbers, physician and clinician names, clinical information, diagnostic information and treatment information. Nearly one year after the breach, on April 14, 2025, ASHN began mailing notifying affected individuals for whom they had valid mailing addresses.
