Chord Specialty Dental Partners (“Chord”) is a provider of business and operational support services to sixty dental practices located in Indiana, Delaware, Pennsylvania , Tennessee and Virginia. More than 173,000 patients had their identity put at risk when an unauthorized character gained access to employees’ email accounts between August 19, 2024 and September 25, 2024. The unauthorized access compromised patients’ personally identifiable information and protected health information including their names, addresses, Social Security numbers, driver’s license numbers, bank account information, payment card information, dates of birth, medical information, and health insurance information. Notification letters were sent to affected patients on March 14, 2024.
