Premier Health Data Breach Investigation

Premier Health is a large medical organization in Southwest Ohio. In addition to five hospitals, the organization includes urgent care centers, emergency centers, primary care practices, outpatient surgery centers, imaging centers, and rehab centers. On July 18, 2025, Premier Health issued a press release about a data breach that occurred in 2023. Specifically, an unauthorized character breached its network between June 7, 2023 and July 12, 2023, which Premier Health discovered on July 12, 2023. Because of the breach, personally identifiable information (“PII”) and protected health information (“PHI”) of patients and possibly employees were compromised. The PII and PHI included names, dates of birth, Social Security numbers, driver’s license numbers, state issued identification numbers, passport numbers, individual taxpayer identification numbers, digital signatures, login credentials, financial account information, medical information, and health insurance information. According to Premier Health’s website, the organization is sending letters to impacted individuals where address information is available.
