DaVita, Inc. Data Breach Investigation

More than four months after the breach, DaVita, Inc. has begun sending notification letters to 900,000 individuals whose personally identified (“PII”) and protected health information (“PHI”) were stolen. According to the notification letter, the following PII and PHI was compromised: name, address, date of birth, social security number, health insurance-related information, and other identifiers internal to DaVita, as well as certain clinical information, such as health condition, other treatment information, certain dialysis lab test results, and, for some individuals, the information included tax identification numbers, and in limited cases images of checks written to DaVita.
