Community Health Network Data Breach Investigation

Community Health Network offers a wide array of medical services to residents of Central Indiana. Its health services include, but is not limited to, primary care, surgical services, emergency medicine, Quest Diagnostics, medical imaging, physical therapy and rehab. On September 12, 2025, the network began issuing notification letters to patients whose protected health information was stolen during a data breach. The breach occurred between February 25 and February 26, 2025 when an unauthorized actor accessed an employee email account. The compromised patient information included patient names, medical information, and health insurance information.
