Cornwell Quality Tools Data Breach Investigation

On or about December 12, 2024, the computer network of Cornwell Quality Tools (“Cornwell”) was breached when an unauthorized party gained access to Cornwell’s system. As a result of the breach, personally identifiable information (PII) and protected health information (PHI) belonging to 103,782 individuals was compromised. The compromised information included the individuals’ names, Social Security Numbers, medical information, and financial account numbers. Affected individuals were notified about the breach by a mailing dated September 4, 2024.
