Outcomes One, Inc. (“Outcomes”) is a third-party service provider to pharmacies. Among other services, pharmacies utilize Outcomes’ digital services to provide patients with reminders and encourage prescription adherence. On or about July 1, 2025, an Outcomes employee fell victim to a phishing attempt that exposed nearly 150,000 patients’ personally identifiable information (“PII”) and protected health information (“PHI”). The PII and PHI included names, dates of birth, genders, phone numbers and/or states, health insurance information (including health plan names), service dates, health care IDs, diagnoses, and/or other medication information. On October 24, 2025, some state agencies received the notification letter that’s expected to be sent by Outcomes to affected individuals.
