SimonMed Imaging Data Breach Investigation

With over 170 facilities across 10 states, SimonMed Imaging (“SimonMed’) is one of the largest outpatient medical imaging providers and physician radiology practices in the nation. On October 10, 2025, the practice began notifying 1.2 million SimonMed patients and patients of facilities now operated by SimonMed about a data breach. A vendor contacted SimonMed on January 27, 2025 to inform them about a breach of the vendor’s systems. The notification prompted SimonMed to investigate its own systems, and the investigation revealed that its own system had been accessed by an unauthorized person between January 21 and February 2, 2025. Due to the breach, patients’ personally identifiable information (“PII”) and protected health information (“PHI”) was compromised. The compromised information included names, addresses, birth dates, dates of service, provider names, medical record numbers, patient numbers, medical conditions, diagnoses and/or treatment information, medications, health insurance information, and driver’s license numbers.
