TriZetto Provider Solutions (“TriZetto”)—a subsidiary of Cognizant Technology Solutions—supplies healthcare providers with revenue management services. An unauthorized actor breached TriZetto’s internal systems in November of 2024, and TriZetto did not discover the breach until October 2, 2025. This incident jeopardizes the personally identifiable information (“PII”) and personal health information (“PHI”) of potentially thousands of patients and employees. Compromised PII and PHI may include, but are not limited to, patient and primary insured names, addresses, dates of birth, Social Security Numbers, health insurance member ID numbers, health insurer information, and information related to dependents. TriZetto has yet to disclose the scope of this data breach. As of January 8, 2026, the following providers are publicly known to be impacted by the incident:
