QualDerm Partners, LLC

QualDerm Partners, LLC (“QualDerm”), is a healthcare management services provider based in Tennessee, who assists 158 dermatology & skin care practices across 17 states. On February 24, 2026, QualDerm reported a data security incident to the Texas Attorney General’s Office. According to their notice, an investigation discovered that unauthorized activity on QualDerm’s internal networks occurred between December 23, 2025, and December 24, 2025. This breach has impacted the personally identifiable information (“PII”) and personal health information (“PHI”) of at least 174,837 current and former patients. PII and PHI includes, but is not limited to: Patient Name, Date of Birth, Doctor Name, Medical Record Number, Date of Death, Email Address, Treatment Information, Diagnosis Information, and Health Insurance Information. Additionally, some patients’ government-issued ID information was included in breached information. This may include Driver’s License Number and/or Social Security Number.
