BW Homecare Holdings, LLC, doing business as Elara Caring, is one of the largest providers of home-based healthcare in the United States. One of its third-party vendors recently experienced a data security incident, potentially exposing sensitive patient information.
On December 12, 2025, an undisclosed third-party vendor assisting Elara Caring with document management informed Elara Caring that an unauthorized actor accessed the vendor’s network environment. Between November 4 and 6, 2025 and November 14 and 17, 2025, this unauthorized actor downloaded and/or accessed documents associated with Elara Caring patients. Certain documents contained an undisclosed number of patients’ Personally Identifiable Information (“PII”) and Protected Health Information (“PHI”). The compromised PII and PHI includes but is not limited to Social Security numbers and medical records, according to the Commonwealth of Massachusetts’s 2026 Data Breach Notification Report.
