If you were a patient at “UBH” from March 2012-November 2022, your personally identifiable information (“PII”) and protected health information (“PHI”) may be at risk. On May 10, 2023, “UBH” began notifying 103,974 affected persons of a breach that occurred in November 2022. The breach exposed patients’ names, dates of birth, Social Security Numbers, addresses, health insurance information and certain clinical details including diagnosis/conditions, medications, test results, and procedure information.
