City of Hope Data Breach Investigation

City of Hope is one of the largest cancer research and treatment organizations in the United States with centers near Atlanta, Chicago, Los Angeles, Orange County and Phoenix. Between September 19, 2023, and October 12, 2023, an unauthorized character accessed its systems. The breach impacted individuals’ personally identifiable information (“PII”) and protected health information (“PHI”) including name, email address, phone number, date of birth, social security number, driver’s license or other government identification, bank account number and/or credit card details, health insurance information, medical records and information about medical history and/or associated conditions, and/or unique identifiers to associate individuals with City of Hope (e.g., medical record number). Though the breach was discovered on or about October 13, 2023, notification letters were not sent to affected individuals until December 14, 2023, and April 2, 2024.
