Family Vision of Anderson, P.A. (South Carolina)

On or about May 21, 2023, Family Vision of Anderson experienced a data breach in which personally identifiable information and protected health information of 62,631 individuals was exposed. The breach was detected on May 22, 2023, and notification was sent to compromised individuals on July 26, 2023. The compromised information included individuals’ name, Social Security number, driver’s license number, date of birth, address, telephone number, email address, gender, medical record number, health insurance company and number, and medical history.
