Fairfax Oral and Maxillofacial Surgery Data Breach Investigation

Between May 15 and May 16, 2023, an unauthorized character accessed the computer systems of Fairfax Oral and Maxillofacial Surgery compromising 235,931 patients’ personal information. The personal information included patients’ names, driver’s license numbers, Social Security numbers, health insurance information, and/or medical information. Though the practice learned about the breach on or about the same date the system was accessed, notification letters were not sent to patients until August 18, 2023, August 21, 2023, and October 12, 2023.
